ADA grievance form
Use this form if you believe the City has not provided equal access to a program, service, activity, facility, website, or document. This is the City's grievance procedure under 28 CFR 35.107(b). You may also file by phone, mail, or in person, or ask for help completing it.
What happens next: the ADA Coordinator acknowledges your grievance, may contact you to discuss it, and responds in writing with the City's position and any remedy. If you disagree, you can request review by the City Manager. Filing a grievance with the City does not limit your right to file with the U.S. Department of Justice.
